La prediabetes tipo 2 (PDT2) es una entidad de alta prevalencia que representa un desafío para la salud pública dado que, al igual que la diabetes mellitus tipo 2 (DM2), tiene un inicio solapado y afecta a millones de personas a nivel mundial. Más allá de su posible progresión a DM2, esta condición se asocia intrínsecamente con un alto riesgo de desarrollar complicaciones, tanto macro como microvasculares, sugiriendo que la separación entre PDT2 y DM2 podría estar actualmente sostenida por un umbral arbitrario en el continuo de la disglucemia desde la normalidad hasta estadios avanzados.
Es imperioso detectar e intervenir precozmente, tanto en la población pediátrica como en la adulta, fundamentalmente con cambios en el estilo de vida y eventualmente con fármacos que hayan demostrado eficacia en estudios debidamente desarrollados. La evidencia sostiene que las complicaciones crónicas y el compromiso multiorgánico pueden ocurrir desde etapas tempranas, se progrese o no a DM2.
